Participant Waiver/Release Agreement

By participating in Hospice Peterborough’s Hike for Hospice event, hosted at a venue owned by The Corporation of the City of Peterborough, I waive and release any and all claims for myself, heirs, executors and administrators against all sponsors, officials and organizers of Hike for Hospice and The Corporation of the City of Peterborough for injury, illness or death which may directly or indirectly result from my participation in the Hike for Hospice event.

I hereby give Hospice Peterborough, its assigns, licensees and legal representatives the irrevocable right to use your photograph/image/name (“My Image”) in all forms and manner including but not limited to publication on print materials, internet websites, social media, broadcasts and any other publications as released to or by Hospice Peterborough. I understand that Hospice Peterborough cannot control unauthorized use of My Image by persons not associated with Hospice Peterborough once My Image has been published. I hereby forever waive any right to inspect or approve any publication of My Image by Hospice Peterborough. I have carefully reviewed and understand the above provisions and agree to be bound by them. I voluntarily and irrevocably give my consent and agree to this Release and Waiver.
